Avoid displaying files with a nil state in vc-dir.
* lisp/vc-dir.el (vc-dir-update): Obey the noinsert argument in all
cases that cause insertion.
(vc-dir-resynch-file): Tell vc-dir-update to avoid inserting files
with a nil state.
